Softfootball understands that AC Milan striker Santiago Giménez is facing a tough decision about surgery for a stubborn ankle problem that’s kept him out for more than six weeks.

As reported by Fabrizio Romano via his official X account, both Giménez and the club staff are seriously weighing surgery as the best way to finally fix the issue.

Giménez has been absent since October, when he was subbed off in the club’s 1-1 draw against Atalanta after the ankle flared up again. The injury first came up during the summer Gold Cup with Mexico.

Giménez went to Holland for a specialist exam. No official word yet from Milan on the findings, but insiders hint the injury’s more complicated than anyone thought. Pain continues, and with no clear recovery timetable, surgery is on the table as a more permanent fix.

Recent reports say Giménez is getting psychological support too, showing how frustrated he is with the situation. Some are starting to wonder if a January 2026 transfer could happen. His form hasn’t helped either, and that’s added scrutiny about his role in Allegri’s plans.

With Giménez uncertain, Milan are already looking at options. Talks with West Ham’s Niclas Füllkrug are ongoing.

Milan head coach Allegri might have to rethink his 3-5-2, maybe switching to a 3-4-3 with Christian Pulisic and Rafael Leão supporting the lone striker if Giménez ends up having surgery and faces a long-term absence. The next few weeks could be decisive, not just for Giménez, but for Milan’s entire attack this season.
